I am writing to provide a summary of the health history and current status of Mr. Adam Cornelius, a 37 year old male who has been under my care.

The patient is generally in good health and maintains appropriate preventive care and routine follow ups. His past medical history is notable for testicular cancer, for which he underwent medical and surgical management in 2021. He recovered well from the procedure and has continued appropriate surveillance with the urology team since that time. There is no current evidence of disease recurrence based on available follow up evaluations.

He reports no chronic medical conditions, takes no daily medications aside from occasional over the counter agents as needed, and has no limitations in his daily activities. His recent physical examinations have been unremarkable, and his vital signs and laboratory studies remain within normal ranges.

Overall, the patient is in good health with a stable post surgical history. He will continue with routine monitoring as clinically indicated.
