The Procedure for Provisional Balloting in Chicago

Should you encounter unforeseen discrepancies at your designated polling location—such as the unexpected absence of your name upon the electoral roll, a lack of requisite identification for same-day voter registration, or the inability to remit a previously requested mail-in ballot—you retain the irrevocable right to cast a provisional ballot.

The Official Protocol:

  1. Converse with an Election Authority: Present yourself at your assigned precinct and explicitly inform the attending Election Judge of your need to utilize a provisional ballot.
  2. Execute the Affidavit: The presiding Election Judge shall dispense an "Affidavit of Provisional Voter." It is incumbent upon you to complete and affix your signature to this document, formally attesting to your legal eligibility to participate in that precinct's election.
  3. Cast Your Vote: You shall be entrusted with a provisional paper ballot alongside a secrecy envelope. Proceed to the voting booth and designate your selections as is customary.
  4. Seal and Relinquish: Securely place your completed ballot within the provisional envelope, seal it thoroughly, and return it directly to the Election Judge. Under no circumstances should this ballot be fed into the standard optical scanner.
  5. Retain Your Receipt: The Election Judge will provide a formal receipt bearing a unique, alphanumeric tracking code. Commencing two weeks following the election, you may utilize this code to ascertain whether your vote was successfully tabulated.
  6. The Vital Follow-Up: If your provisional vote was necessitated by a lack of proper identification, it is imperative that you present the required documentation to the Chicago Board of Election Commissioners (69 W. Washington St., Suite 600) within 7 calendar days subsequent to Election Day. Failure to comply will result in the nullification of your ballot.
